Health examination form

All athletes must have a valid health form on file in order to compete. Health exams are only valid for one year and must be renewed each year.

 

Permission form for Athletic Field Trips

All athletes must have this form on file with their coach. A new form must be filled out for each season of sport.

 

DMV Clearance

Any parent who wishes to drive athletes must have a DMV clearance on file. Fill out the form and take to the DMV and then return the completed report to the Athletic Director.

 

Mercy Driving Form

This form explains the policy on use of private cars for transporting students. The form must include a copy of your drivers license, insurance information and vehicle information.
