Important Forms

Below are some of the important forms we need completed and on file for all students. These forms are included in your orientation packages given to you at the beginning of the school year. Should you need a replacement form or to update your information, please download and print the form you need and mail or fax us (415) 334-9726 the completed form.

 

Medical Form	Students must receive a physical examination by a licensed physician and have the physician complete this form. Every student is required to have a current medical form filed with us. Medical forms are only valid for 1 year from the date it was signed by the physician. Therefore, a new form is required each year. Athletes must have a current Medical Form on file with the Athletic Director prior to participating in sports.
Emergency Contact Card	In the event of an emergency or disaster we must be able to contact a student's parent, guardian, or other designated emergency contacts. It is very important that this form be fully completed, current, and on file with us at the start of each year.
Athletics Forms	All athletes are required to have a completed health form as well as a field trip permission form in order to participate in sports. These forms, as well as DMV clearance and driving forms for parents, are located in the Athletics section.
